Access to Your Records
Current and former clients have the right to request and receive a copy of their mental health records maintained by Shadow and Sun Counseling. Records will be provided in accordance with Texas law and abiding by applicable privacy regulations.
How to Request: To request your records, please submit a written request to Shadow and Sun Counseling via email and include full name, description of records being requested and dates of treatment as well as preferred method to receive records.
Legal Compliance: This process is in accordance with Texas Health & Safety Code §611.0045.
Timeline: Your provider has 15 days to respond to your written request, as required by Texas Health & Safety Code §611.008(a).
